How to Apply for Funding

The Komen Mid-Michigan Affiliate is proud to announce the availiblity of grant funds for 2012-2013.  Grants will be awarded to support efforts to reduce breast cancer disparities in Ingham, Eaton, Clinton, Jackson, Shiawassee, Livingston and Washtenaw Counties.  Based on the community needs assessment, Komen Mid-Michigan has identified the following funding priority areas:

1. Promote breast health awareness, the importance of screening and improving the continuum of care for women in our 7 county service area.
2. Provide eligible women (in the 7 county service area) who are uninsured or underinsured, ages 40-49, with the ability to obtain a potentially life saving mammogram. 
3. Provide breast cancer survivors with programs offering emotional, social and physical support during and following breast cancer treatments. 

Potential Applicant will need to download and complete ALL of the following documents. 

The deadline for grant submission is 9am on Monday January 9th. 

ALL completed applications MUST be received by January 9th to be considered.
